- Invoices and billing. You will receive invoices by email. The CPE Charge is due upfront on signature of this Agreement. The MRC will be billed a month in advance, beginning on or after service activation (“In-Service Date”).
- Accuracy. The information I provided is true and accurate to the best of my knowledge.
- Two‑year commitment and promotional pricing. I am receiving promotional pricing on the CPE Charge and MRC in exchange for my two‑year commitment. After the initial two-year term, service converts to month‑to‑month at then‑current rates, and you may cancel by providing notice before the next billing cycle. You may request to increase or decrease my speed during my two-year term by contacting Choice Customer Care, but the promotional pricing may not be available at that speed and any change will not alter the number of months left in your remaining two-year commitment, if any.
- No early termination. If I cancel during the initial term or breach this Agreement or any of the Additional Terms (referenced below), Choice may terminate this Agreement, and I will owe an early termination fee equal to the remaining monthly MRC charges through the end of the two-year term plus any fees or costs incurred by Choice in recovery those amounts from you, to the fullest extent permitted by applicable law. Any collection costs and attorneys’ fees will be limited to what is reasonable and legally allowable.
- CPE ownership and Equipment Return. All Customer Premise Equipment (“CPE”) remains the property of Choice. The CPE Charge and MRC fees do not constitute a purchase, or transfer, of ownership of the CPE. You are responsible for any CPE that is damaged beyond normal wear and tear, vandalized, or remains unreturned after termination or expiration of this Agreement. Upon termination or expiration of this Agreement, you must promptly contact Choice to schedule an equipment return.
- Monthly payment obligation and auto‑pay authorization. The MRC is due each month regardless of usage. If I provide a payment method for auto‑pay, I authorize Choice to charge that payment method for recurring monthly MRC until I revoke authorization in accordance with Choice’s stated payment procedures.
- Late payment; suspension and termination for payment default. If payment is late, Choice may suspend service after providing notice. Service will be restored after all past‑due MRC amounts and any applicable reconnection fee are paid. If service remains suspended for three (3) consecutive months, Choice may, at its discretion, terminate this Agreement in accordance with the terms and conditions herein (see section 4 above).
- Equipment condition and replacement cost. If recovered equipment is damaged beyond normal wear and tear, vandalized or cannot be recovered, I agree to pay the actual replacement cost of the CPE. If I haven’t returned CPE within sixty (60) days from termination or expiry of this Agreement, Choice reserves the right to charge you for the actual replacement cost of the CPE. If a payment method is on file, I authorize Choice to charge that amount; otherwise, I will pay upon invoice.
- Failed installation. If Choice cannot confirm service availability at my address during installation, I will receive a full refund of amounts paid for service activation.
- Monthly Equipment Fee: $10.00 per month
- Insufficient Funds on returned checks - $35.00
- Reconnection Fee - $25.00 – MRC will still accrue and the two-year term will toll during any suspension period
- Relocation / move service - $99.00
- Paper invoices are available for a $2.50 additional charge.
- See Fees & Surcharges on our website (Fees & Surcharges - Choice Wireless) for additional information that may be applicable to your bill.
